Legal Aid Assessment for Mediation – Client only

During your assessment meeting we will complete the Legal Aid Assessment form with you [this is referred to as a Means 7 form]. As this covers your personal information [Income and capital details] we do not complete this form fully through the website because we take protecting your personal information and our responsibilities under the Data Protection Regulations seriously.

Once we have asked you a series of questions and completed the form, we will send you this via email for your verification, approval and consent to the terms. You will see on page 6 of the form we email you this lists the evidence we require in support of your application. This is explained further on the website in our Legal Aid page information sheets and you can email any evidence you need to on this page.

You can either download this client declaration, print, sign and post back to us or you are able to sign online, using your mouse/Trackpad on your PC/laptop or your finger on mobile devices. Please complete the form fields, these will populate into the correct areas of the form and will be sent to us electronically. You will receive an email with a copy of the client declaration for your reference.

We will ensure we keep a fully completed Means 7 including your signed declaration to satisfy the Legal Aid Agency’s requirements so we can recover your costs from them and be able to offer you a free service.
